Indications and Dosage. Intravenous. Susceptible Gram-negative infections, Susceptible Gram-positive infections, Susceptible infections. Adult: 200 mg on the 1st day given in 1 or 2 infusions, followed by 100-200 mg daily depending on the severity of the infection. Doses are given via infusion over 1-4 hours (depending on the dose).
Child: ≥6 months As immediate release suspension: 10 mg/kg daily for 3 days or 10 mg/kg on day 1, followed by 5 mg/kg/day on days 2-5. >45 kg: Same as adult dose. Adult: 1 g or 2 g as a single dose, in combination with ceftriaxone. Child: >2 years As immediate release preparation: 12 mg/kg/day for 5 days.
Dosage/Direction for Use. FC tab Adult & childn >12 yr 1 g bd. Mild to moderate infections 375 mg tds. More severe infections, resp tract infections 625 mg tds. Oral susp Childn 7-12 yr 312.5 mg tds, 2-7 yr 156.25 mg tds, 9 mth to 2 yr 78.12 mg tds. 457 mg/5 mL oral susp Childn 2 mth-12 yr Mild to moderate infections 25/3.6 mg/kg/day.

Drug Interactions. Increased metabolism w/ substances known to induce drug-metabolising enzymes (specifically CYP450 enzymes) eg, anticonvulsants (eg, phenobarb, phenytoin, carbamazepine) & anti-infectives (eg, rifampicin, rifabutin, nevirapine, efavirenz); ritonavir, telaprevir & nelfinavir; herbal prep containing St. John's Wort.
Dosage/Direction for Use. Adult 2 tab daily, morning & noon. In severe cases, morning dose may be increased to 2 tab. Max: 4 tab daily. Elderly >65 yr 1 tab in the morning.
