Nothing else. Do NOT use this site. Only use the official site: https://evisa.xuatnhapcanh.gov.vn/. it's $25.00 USD.

Disclosing whether you have arranged insurance is required with only Yes/No options. Saying No does not prevent you from submitting the application as will omitting any information marked with the asterisk. JD, it doesn't mean insurance has suddenly become compulsory, just disclosing whether you have it or not at the time of application.

I had a e-visa approved by Vietnam immigration but when I arrived at the airport they pointed out I had left off my middle name and would not let me board.

If you need a visa to enter Vietnam, you will not be allowed to board a plane to Vietnam without one, Unless you are entitled to an exemption, then an E-Visa https://immigration.gov.vn is what you want. If you are after a VOA visa, you'd be better off applying via one of the Vietnam agents, such as vietnamvisapro. They are legit (you pay by card) and usually get your approval letter within 48 hours - frequently much less.
