SC Primary hypercholesterolaemia & mixed dyslipidaemia (including HeFH) Adult & paed ≥10 yr 140 mg every 2 wk or 420 mg once mthly.HoFH Adult & paed ≥10 yr Initially 420 mg once mthly, can be up-titrated to 420 mg once every 2 wk if clinically meaningful response is not achieved after 12 wk. Patient on apheresis May initiate treatment w/ 420 mg every 2 wk to correspond w/ apheresis schedule.
Treatment duration: 2-5 days for transient insomnia; 2-3 wk for short-term insomnia. Do not exceed 4 wk, including tapering-off period. Adult 10 mg single dose taken immediately at bedtime. Max dose: 10 mg. Elderly or debilitated patient 5 mg (½ tab). Patient w/ liver failure Initially 5 mg daily. Max dose: 10 mg.
Vaccine adjuvant 900 mcg/m 2 (1.6 mg) twice wkly w/ 3-4 days interval & 1st dose given coincident w/ vaccine. Continue for 4 wk (total: 8 doses) after a single-dose vaccine immunisation regimen. For multidose vaccine immunisation regimen, continue twice wkly between vaccinations & for 3 wk (total: 5-6 doses) after last vaccination.

Max: 90 mg daily. Acute gouty arthritis 120 mg once daily, limited to max of 8 days. Post-op dental surgery pain 90 mg once daily, limited to max of 3 days. Patient w/ mild hepatic impairment (Child-Pugh score 5-6) Max: 60 mg once daily. Patient w/ moderate hepatic impairment (Child-Pugh score 7-9) Max: 30 mg once daily.
