長者或其家人可透過社會福利署或非政府機構轄下的綜合家庭服務中心、社會福利署或醫院管理局轄下的醫務社會服務部、長者地區中心、長者鄰舍中心、長者活動中心或其他服務單位（如保護家庭及兒童服務課）提出申請或要求轉介。

申請手續 申請人填寫「服務申請表」，並提交申請人及同時申請本服務之家人的身份證明文件、近2個月之住址證明文件及同住所有家庭成員最近6個月之收入證明文件(包括家庭經濟支柱)及資產證明文件，接受評估人員面見以作審批。
