As you have done Phuket before, Samui or even Krabi could be a good alternative. With nearly three weeks, that is plenty of time to explore and relax at each.

Bangkok is a big, loud, busy, 24 hour city with lots to do and see. 5 days is not a lot of time to be packing in 2 destinations, but you could head to Patong Beach in Phuket ...
