Initially, 25 mg every 2 weeks. For patients on fixed dose of oral risperidone treated with >4 mg daily for ≥2 weeks: Initially, 37.5 mg every 2 weeks. Doses may be increased in increments of 12.5 mg at intervals of at least 4 weeks. Max: 50 mg every 2 weeks. Continue oral supplementation for 3 weeks after the 1st inj.
Adult: Loading dose: 2-4 mg/kg via continuous infusion over 15-20 minutes, given before cancer therapy. Maintenance: 3-5 mg/kg via continuous infusion over 8-12 hours. Alternatively, initial dose of up to 2 mg/kg via intermittent infusion over at least 15 minutes, may be given before cancer therapy and repeated at 2-hourly intervals.

Indications and Dosage. Adult: As extended-release tab: Mild to moderate hypocitraturia (>150 mg/day urinary citrate): Initially, 15 mEq bid or 10 mEq tid. Max: 100 mEq daily. Severe hypocitraturia (<150 mg/day urinary citrate): Initially, 30 mEq bid or 20 mEq tid. Alternatively, 15 mEq 4 times daily.
IV Infusion Initially, 100 mg in 500 mL infusion fluid w/ starting infusion rate of 0.5 mL/min (10 drops/min). Increase infusion rate in increments of 10 drops/min at 10 min interval. IM inj 10 mg every 3 hr for 24 hr then 4-6 hr for 48 hr. Circulatory disturbances Oral 20 mg tid-qid. Inj 5-10 mg IM tid.
