(如續領)未剪角的舊BNO護照；如你的護照已遺失或有損毀，則而另外填妥「Cancel a lost or stolen passport」表格。 BNO以外的有效護照全彩色影印本，每一頁都需要影印；或提供該護照的正本。 ＊註︰97年後出生港人、上述新安排下的申請文件要求，尚待正式宣布。
