In general, the past simple describes a completed action while the past continuous describes an interrupted action in some way - that is, one which happens around another action or time. Wear is an unusual verb in that it is usually in the continuous unless you are talking about a particular event or situation.
Level: intermediate. The past perfect is made from the verb had and the past participle of a verb: I had finished the work. She had gone. The past perfect continuous is made from had been and the -ing form of a verb: I had been working there for a year. They had been painting the bedroom.
So firstly, simple past tense simply refers to an action or event which happened (i.e. began and ended) in the past, correct? Secondly, "happened" equals "began and ended"? Thirdly, if I say something happened, by default, it means this thing started and ended in the past?

Firstly, it is my understanding that when we use the past participle in this way, it is no longer the past participle but the 'passive verb'. The past participle always follows a form of the verb 'to have', but the passive verb always follows a form of the verb 'to be'.
