Patient w/ mild to moderate hepatic impairment Start therapy at reduced number of doses. Max: 2 tab daily. Patient w/ mild renal impairment (CrCl 60-89 mL/min) 2 tab daily.

H. pylorieradication to reduce risk of duodenal ulcer recurrence Triple-therapy: 30 mg bd + amoxicillin 1 g bd + clarithromycin 500 mg bd for 10-14 days. Dual-therapy: 30 mg tds + amoxicillin 1 g tds for 14 days.
